Enabling and Using COVID-19 Mobile Form

Many states/regions are recommending or requiring that employers ensure their employees are healthy prior to work to help slow the spread of COVID-19, also known as Coronavirus. To help with this, we have begun offering a series of ExakTime Mobile Form questions that all your employees will be prompted to answer upon Clock In.

 ExakTime Mobile 2.33+ Required

For this functionality, it will require a minimum ExakTime Mobile version of 2.33. It is recommended to update to the latest publicly available version.

How It Works

The COVID-19 Mobile Form is a series of questions presented on ExakTime Mobile as employees clock in for themselves with the "Clock In" function. After your employees answer their questions and clock in, they are sent/synced to ExakTime Connect with the associated time record for further review by an Administrator.

Whenever an employee clocks in, the first question that will be asked is:

  • Is this your first clock in of the day?
    • Possible Answers: Yes or No

If an employee answers "No", they will clock in as normal. If they answer "Yes", they will be presented the following questions/statements:

  • Have you had a fever, dry cough or trouble breathing in the last 24 hours?
    • Possible Answers: Yes or No
  • Have you had close contact with anyone diagnosed with COVID-19 or been asked to self-isolate by a doctor or health official?
    • Possible Answers: Yes or No
  • If you answered yes to any of the previous questions, please leave the jobsite and contact your supervisor after clicking the Next button below.

Enabling COVID-19 Questions

Administrators can enable these COVID-19 questions in ExakTime Connect through Company Settings.

  • Go to Manage and click Company Settings.


  • Click the Other tab.


  • Scroll down for the ExakTime Mobile Form options and enable the "COVID-19 Mobile Health Screening Form" option*.


  • Click Save at the bottom.

Your employees will need to sync their copies of ExakTime Mobile before they will be presented with the COVID-19. It is advised to double-check with employees that they are presented with these questions and if there is any trouble, feel free to contact us here for help.

*If you do not see the options from the previous image and instead see the following text due to having an existing custom form, you will need to reach out to your sales executive/account manager.

What Employees Will See

As employees clock in throughout the day, they will be prompted with the question "Is this your first clock in for the day?" If they answer yes, they will proceed to the next questions. If they answer no, the following questions are skipped instead.


How to Review Employee Responses

After an employee answers and submits their questions, an administrator can review the responses from ExakTime Connect. They can be reviewed from three locations, the time card detail page, ExakTime Forms Viewer page, or with the ExakTime Mobile Forms report. Examples of each have been provided below.

Time Card Detail

To view the ExakTime Mobile form responses on your time card detail page, you must enable the option it as it is not enabled by default.
