Premade Forms for ExakTime

While you can create your own forms for ExakTime, we also offer several default forms to help you get started and take advantage of other features/modules we offer. The questions, names, and pages of these default forms cannot be modified as other features/modules depend on these pre-made questions and pages.

Premade Forms

These default forms can be found in the Form Manager and will be inactive by default. The forms must be made active for your employees to use.

Premade Form Limitations

When editing a form, you can only assign a form and make it active as the form may be associated with other reporting and features.

Employee Workplace Incident Report

The 'Employee Workplace Incident Report' form allows your employees to provide an incident report when they clock out. The form is available as an 'Arcoro Clock Out' form for your employees. 

Your employee's responses can be reviewed from ExakTime Connect with methods such as the Response Viewer or reports.

If you also use Advanced Analytics, you can use the related Safety-focused Dashboards to view advanced reporting and visualizations of your employee's responses.

Supervisor Workplace Incident Report

The 'Supervisor Workplace Incident Report' form allows employees to provide an incident report from ExakTime Mobile. The form is available as an 'Arcoro Library' form for your employees to complete. 

Your employee's responses can be reviewed from ExakTime Connect with methods such as the Response Viewer or reports.

If you also use Advanced Analytics, you can use the related Safety-focused Dashboards to view advanced reporting and visualizations of your employee's responses.
