Affordable Care Act - View Completed Forms

On the View Completed Forms tab, you can view and/or eFile any forms that were generated on the Generate Forms tab.

  • The Completed Forms tab can be found by going to the menu, expanding Settings and clicking Affordable Care Act.

  • Click Forms from the top menu.

  • Click the View Completed Forms tab.

  • The "View Completed Forms" page will show any previously created forms and their status.

    • Form & Employers:
      • The selected Form that was generated.
      • The year the form was generated for.
      • The Employer and Employee Group.
    • Submission Status:
      • Status of submitted forms.
      • Generated Date: The date in which the form was generated.
      • Generated By: The name of the administrator who generated the form.
      • eFiled Date: The date in which the form was eFiled with the IRS.
      • eFiled By: The name of the administrator who eFiled the form.
    • Merged 1094/1095: The generated form as an Excel file.
    • 1094: The generated form is available as both a PDF and Excel file.
    • 1095: The generated form is available as both a PDF and Excel file.
    • Show on employee Portal: Will allow you to make the forms available on the employees portal under the links sections.

Once a form has been generated, select the checkbox for the form you wish to eFile. From the Actions menu, click Process Form Now. Once the form has been eFiled, the eFile Date and eFiled By columns will generate a value.
